LICENSED PRACTICAL NURSE (LPN) DESCRIPTION:

The LPN provides direct supervision for nursing personnel during his/her work shift, which includes providing input to performance evaluations and disciplinary processes. He/she ensures that nursing care is provided according to standards and policies of the facility and current nursing principles and techniques. The LPN treats residents, family, and team members with dignity and respect. He/she identifies resident needs and helps develop and implement individualized plans of care in order to maximize the quality of life for each resident. 

 

LICENSED PRACTICAL NURSE (LPN) RESPONSIBILITIES:

Assumes responsibility for administering quality care to meet the total needs of residents, receiving direction from the Care Coordinator and the Director of Nursing, Administrator of Health Services, or Director of Human Development & Engagement as required.

Communicates individualized plans of care to CNA, CMA, and other appropriate staff, and ensures individualized care according to residents' care plan as ordered by the physician.

Prioritizes resident care according to need and acuity.

Notifies appropriate staff when a resident is transferred to the emergency room, hospital, or in the event of death.

Oversees routine, stat, and incidental laboratory and X-ray requests, including requisitions.

Notifies the attending physician promptly of changes in residents condition, unusual occurrences, medication reactions, medication errors, or excessive weight changes.

Supervises dispensing of medications and end-of-shift scheduled drug verification.

Charts every shift for residents whose charts have been flagged to identify a particular situation or problem that must be monitored closely.

Completes all required documentation according to Epworth Villa protocol and communicates information to appropriate personnel.

Maintains residents' privacy and confidentiality of records, services, and communications according to established protocol.

Participates in new resident and family orientation.

Serves as the primary nursing staff liaison with resident families.

Ensures prompt response to call signals.

Ensures CMAs reorder resident medications within five (5) days of running out of medication.

Gives and receives report at shift change that includes all pertinent information.

Prepares, administers, and charts medications and treatments, if needed; provides tube and tube med-administration feeding care; and provides IV services as ordered by physician.

Makes resident rounds as needed, noting customer condition, comfort, appearance, and condition of surroundings; makes appropriate interventions; participates in quality assurance rounds; and updates residents' plans of care regarding all pertinent changes.

Collaborates with other staff members using team approach to maintain/increase the quality of life of residents.

Provides leadership and supervision for the nursing staff on assigned shift, including CNAs, CMAs, Ward Clerk, and Restorative Aides; treats team members with dignity and respect; provides input to the Care Coordinator and participates in performance evaluations for each person under his/her supervision; provides first-line supervisory discipline to any person directly supervises; documents specific details to ensure accurate and timely reporting of the situation to the Care Coordinator.

Helps orient and train new personnel and serves as a working supervisor helping CNAs and CMAs with daily supportive care.

Attends all staff in-service training and participates in job–related continuing education as required for this position.

Practices safety at all times and applies all accident, fire safety, or emergency procedures of the facility in compliance with Epworth Villa Safety Policies and Procedures.

Provides input to the Care Coordinator for performance appraisals, rewarding, and disciplining employees and helps address complaints and resolve problems.

Performs other duties as assigned.

LICENSED PRACTICAL NURSE (LPN) QUALIFICATIONS:

Must be a Licensed Practical Nurse (LPN), or Registered Nurse (RN) (preferred).

At least two years of satisfactory work experience as a nurse, satisfactorily demonstrating “charge nurse” skills that are characteristic of excellence in leadership, communication, assigning and delegating, problem solving, decision making, and facilitating change management.

Current CPR/First Aid certification through the American Heart Association.

Must be knowledgeable of state and local regulations regarding long term care.

Communication and interpersonal skills to develop relationships with clients, staff, and providers.

Experience in supervising and developing staff.
