We are looking for a compassionate LPN to join our Hospice team!

Competitive starting pay of $30.00-$35.00+ per hour (pay DOE)

Epworth at Home has a $1,500 Sign-on bonus for all Full-time LPN new hires.  

  The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse.  Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services or hospice, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST,) to the registered nurse and/or Clinical Supervisor.
  • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.

Job Purpose: To provide compassionate daily living care that enhances the lives of our Home Health and/or Hospice patients.

Skills/Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Has three (3) years nursing experience.  Hospice experience is STRONGLY preferred.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, technical and organization skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Certificates, Licenses, Registrations:

  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Possess and maintain valid CPR certification.
