Your Key Responsibilities:

• Coordinate and implement individualized care plans, revising as necessary.
• Educate patients and their families on disease processes, self-care, and end-of-life care.
• Initiate preventive and rehabilitative nursing procedures.

What We’re Looking For:

• Education & Licensing: Graduate of an accredited nursing program with a current RN license in [State].

• Certifications: Current CPR/First Aid certification from the American Heart Association.

• Transportation: Valid driver’s license, reliable vehicle, and proof of insurance.
